The diagnosis appeared due to a shadowy psychopath who traumatized Pinkamina and four personalities were born in her:Pinkie Pie, Candy and surprise. The latter contained a diagnosis that could contrast with it with the help of foreign objects. That's why Dr. Psycho couldn't get to her because it just couldn't be done.
A party for one[]
In this Creep, after all Pinkie Pie's friends refuse to go to the party, instead of Pinkamina appearing, a surprise appeared in which there was a diagnosis and she took control of her mind. And the first victim was supposed to be Applejack, as her sister says she disappeared from the harvest. After that, the friends go to Pinkie Pie's diagnosis and ask her, to which she replies that she doesn't know. After everyone has left except Rainbow Dash, she stops her and says that this is a prank and wants to show where she is, to which she agrees and she is shown to be killed in a cruel way.And surprise starts abducting other ponies and Pinkie Pie's friends as well. Then she comes to Twilight and says she's afraid she might be abducted, too, and Twilight calms her down and she invites her to her place, to which she agrees.
