Juggernaut's appearance always changes and turns into another killer but in it's appearance in game modes thumbnail is a light green humanoid creature (like Zombie) with red-white eyes, a dark green tie and a dark green hat with light green line.

Almost Nothing known about its past but there is theories.
In theories, It may be created after Players successfully killed all killers and rescued the Area 51 in Classic/Killer and Boss Rush Mode.
Other theories is that it was actually hiding in some room/place in Classic/Killer and Boss Rush Mode. Some times later it went out from it's hiding place and captured Area 51.
Juggernaut Mode[]
In Juggernaut Mode, it and players only person in Area 51, showing that Area 51 is abandoned.
If Juggernaut wins (kills everyone) probably it will escape from Area 51, If Players won, it will get killed.
There is also some theories which is user will sent to "hell" after Juggernaut Mode.
