The Wario Apparition is a character created by Nintendo and popularized by the Mario 64 horror community with a creepypasta based on the theory that every copy of the original Super Mario 64 is personalized, and that there exists a cursed beta version of the game from July 29, 1995. He is the opponent of the song Apparition and Forbidden Star.

Should the description for Forbidden Star be believed, the Wario Apparition started out as Wario himself. Grabbing a midnight snack, Wario's poor night vision led him to accidentally eating a Power Star, turning him into a giant floating head. However, this should likely be taken with a grain of salt, as it is locked behind a song that is deemed non-canon to the mod.
The description in Apparition paints the Wario Apparition as nothing more than a result of the personalization of Super Mario 64, a theory stating that the game changes to suit the player.

After defeating I Hate You Luigi, Boyfriend goes to Wario's world, Girlfriend is nowhere to be found... only for a familiar, disembodied voice to ask if he wants to have some fun. He is then chased down an endless corridor by the Wario Apparition, a monstrous being created by Super Mario 64's customization AI with the intention of granting the wishes of players who wish to play as Wario in the game.

The Wario Apparition is a massive, disembodied, levitating head of Wario from the Mario franchise. Wario has brown hair, blue accents to the sides of his eyes, a pink nose, a zigzagging black mustache, and wears a yellow hat with a blue W on it. In his left pose, his eyes turn into green hypnotic spirals.

The Wario Apparition is a sadistic entity that likes to chase and kill whoever it comes across. Seemingly boastful about what he does, the Wario Apparition occasionally taunts his victims while attacking.

- Wario Apparition's left pose is a reference to a commercial for Super Mario Land 2: 6 Golden Coins, where he tries to hypnotize the viewer to destroy Mario.
- This also happens to be a offical appearance of Wario just as a head.
- Upon losing in Apparition Legacy version, the player is faced with an image of Mario.EXE smiling while Japanese text is displayed. When translated, this roughly reads "Nintendo does not allow unlicensed or pirated products. Remove the cartridge and contact technical support. Game Over"
- Originally, Forbidden Star is found in the files and was scrapped.[1] But, it was added to Freeplay in V2 of Mario's Madness.
- Should the player pause the game during Apparition, the pause menu music will be replaced with the one in Wario World, complete with Wario taunting the player.
- During Apparition, the UI changes to reflect that of an old recording, with the date being that of the computer's, albeit in the year 1996.
- The Wario Apparition has icons, but due to the UI change in V2's rendition of the song, they go unused.
